Common cardiothoracic surgeries and procedures

A few common adults and paediatric cardiothoracic surgeries are,

  • Angioplasty- Percutaneous transluminal coronary angioplasty or balloon angioplasty helps to restore healthy blood flow to the heart by opening the arteries that have been narrowed by fatty plaque. Most people with blocked arteries are asked to undergo angioplasty, depending on the defect in the heart.

  • Pacemakers and implantable cardioverter defibrillators- To rectify abnormal heart rhythms and help the heart beat normally, the doctor may recommend implanting a device. The most common ones are pacemakers and implantable cardioverter defibrillators.

  • Open-Heart Surgery- Open heart surgeries are most commonly done to treat blockages in the coronary arteries or any problems in the heart valves. It can also repair heart defects or heart rhythm problems. It is a detailed procedure and requires a large incision to correct the defect. It is generally done when there is significant damage to the heart and to help the patient live a longer and healthier life.

  • Coronary artery bypass grafting is one of the most commonly performed open-heart surgery. Widely known as “cabbage”, a vein from the leg or an artery from the chest wall or arm is used to bypass a coronary artery that is narrowed or blocked because of plaque build-up. Grafting is a technique used to use the tissues from a patient to help repair the defect in the heart.

  • Heart valve repair or replacement- If a heart is damaged, diseased or fails to open, it strains the heart muscle. Open-heart surgery may be recommended to correct it.  A valve will be repaired and replaced by the surgeon in  open-heart surgery to help it function effectively.

  • Ventricular assist devices- An implantable heart pump is used to treat heart failure.

  • Heart transplant- A heart transplant is used to replace a severely damaged heart with a new heart from an organ donor. This procedure is considered only a last and lengthy alternative that will take some time to fall. The doctor has to perform a detailed diagnosis and look at the parameters to find a suitable donor. 

  • Carotid surgery is a simple surgery that removes plaque build-up in the carotid arteries, the main arteries in the neck that supply blood to the brain.  This is a specific surgery performed in a particular artery to restore blood supply to the brain.

Procedure 

The type of surgery may differ according to the needs of the patient. There are a few things that are common to most of the procedures listed above,

  • Anaesthesia to keep the patient asleep and pain-free during the surgery.

  • The airways are kept open with an endotracheal tube inserted in the throat and windpipe. This is connected to a ventilator, and the ventilator breathes for the patient when asleep. 

  • An incision is made at the middle of the chest, which separates the breastbone. Post-surgery, the surgeon will join the breastbone with heavy sutures or wires and sew the incision.

  • Constant blood circulation with a heart-lung bypass machine to perform the heart and lungs work during the surgery.
