Pre-operative check-up treatment in Hebbal, Bangalore

What is a pre-operative checkup?

Pre-operative check-up treatment in Hebbal, Bangalore or pre-operative evaluation is the process of assessing the ability of a patient to withstand the procedure to be performed.  It is a fundamental part of the perioperative checkup of a patient who is to undergo surgery. The role performed by the anesthesiologists at this time is generally referred to as PAE (pre - anaesthesia evaluation) or PAC (pre - anaesthesia check). Usually, PAC will precede the anaesthesia delivery in case of both nonsurgical and surgical procedures.

Why is it performed?

Pre - anaesthesia evaluation/checkup is performed to assess all known and unknown co-morbidities in the patient that will indirectly or directly impact the patient’s perioperative management. In this regard, if there is appropriate foresight, then specific plans can be chalked out, and their alterations thought out for in-patient management to ensure that the surgery is safer for the patient. PAE / PAC also aid with risk stratification and provide ground for obtaining the highest level of preoperative optimisation.

What are the various benefits of the checkup?

The basic form of PAC comprises the history, pertinent medical records review (if available), and physical examination of the patient.  If any abnormality(s) is/are detected, further investigations and/or consultations will be lined up with doctors specialising in that field.

With optimum PAC, perioperative morbidity and mortality are reduced, requiring needless consultations and investigations. A further benefit of pre-operative checkups is a decrease in delays and case cancellations and a shortening of the required hospital stay. It is a fact that such patients with severe, or even moderate, co-morbidity conditions need preoperative assessment, investigation and management in a most aggressive way so that their surgery can be conducted safely.

What is the procedure for the checkup?

PAC is made of laboratory investigations and medical check-ups.  It is conducted by a competent authority, such as an anaesthesia provider, and performed before the surgery/operation. The essential purpose of PAC is to assess the physical condition of the patient as well as any diseases or medical problems that the patient might be facing.  The assessment’s fundamental goal is to identify all the factors that will significantly raise any risks of complications.  This will decide how the procedure needs to be appropriately changed and modified to counter the threat. The aim is to identify the appropriate anaesthetic techniques to employ so that it is possible to ensure the safety of perioperative care, optimal usage of resources, bettering of outcomes and patient satisfaction while taking into consideration of the person and individual-related risk factors and circumstances. In pre-anaesthetic assessment, information comes from various sources, such as interviews, previous medical records, physical examinations, laboratory tests, and medical examination results.

The following mnemonic is used for pre-anaesthetic assessment so that no aspects get left out.

It is alphabetic for easy recall:

A: Affirmative history and Airway

B: Breathing, blood haemoglobin, estimation of blood loss, blood availability

C: Co-morbidities, clinical examination

D: Drugs that the patient is using, details regarding previous surgeries and anaesthesia

E: Evaluation of all investigations, endpoint for considering surgery go-ahead

F: Fasting, fluid status

G: Give the physical status; get consent for performing the surgery
