Radical surgery is recommended in patients with stage I diseases. Extrapleural pneumonectomy is the standard technique because it provides the best local control; however, in high-risk patients with favourable histology, pleurectomy with decortication may be considered. Long-term survival is recorded in patients who effectively undergo resection with clear margins and no nodal involvement.
There is currently no evidence that radical surgery benefits patients with advanced disease or peritoneal disease. Pleurectomy and decortication may be used to alleviate symptoms such as tumour mass, pain, and recurrent pleural effusions. Pleural sclerosing procedures can be used to treat pleural effusions, but failure rates are high. Following surgical intervention, the addition of radiation therapy and/or chemotherapy has not been shown to improve survival.
Treatment and Prognosis
For oral leiomyosarcoma, radical surgery is the treatment of choice, with adjunctive chemotherapy or radiotherapy used infrequently and with variable results. A radical neck resection is frequently required. The prognosis is generally poor, with numerous recurrences and distant metastases. The precise location of the tumour, particularly in relation to the surgeon's ability to remove it adequately, seems to be crucial in determining the prognosis.
