An abdominal incision is made to reach your uterus and remove fibroids during an abdominal myomectomy (laparotomy).

Laparoscopic Myomectomy, also known as Robotic Myomectomy, is a surgical procedure that removes fibroids via small abdominal incisions. For those suffering from fibroids, a laparoscopic myomectomy is suggested and advised. When a woman wants the fibroids removed but wants to keep her uterus, this procedure is usually recommended. Fibroids disrupt daily life by causing pelvic pain or pressure, heavy menstrual bleeding, urinary frequency, or incontinence.

Why is the procedure performed?

If you have multiple causes and symptoms that interfere with your normal activities, such as heavy menstrual bleeding, pelvic pressure, or urinary incontinence, your doctor may recommend a laparoscopic myomectomy.

Some other reasons why a doctor might suggest the procedure are:

  • If you want to have children, a myomectomy is a good option.

  • If the uterine fibroids are interfering with fertility.

  • If you want to keep your uterus but only have the fibroids removed.

Since fibroids usually interfere with fertility, laparoscopic myomectomy is difficult but has advantages such as shorter hospitalisation time, less postoperative pain, less febrile morbidity, faster recovery, and less blood loss. Laparoscopic myomectomy also reduces the risk of postoperative adhesion formation, and this surgery also reduces the risk of complications in future pregnancies due to uterine rupture. Procedure

In the hours before surgery, one must fast, which implies not drinking fluids or eating anything. Prior to the surgery, the doctor's instructions must be followed. If the patient is already taking medication, it is best to consult with the doctor about whether or not to continue taking the medication.

The procedure begins with giving anaesthesia to the patient, followed by which four small incisions are made by the surgeon. Each incision in the lower abdomen is about half an inch long. The abdomen is filled with carbon dioxide gas, allowing the surgeon to see inside. The surgeon inserts a laparoscope through one of the incisions. A laparoscope is essentially a lighted thin tube with a camera at one end. Small instruments are inserted into the other incision.

In the case of robotic-assisted laparoscopic surgery, the instruments are controlled remotely with a robotic arm. The surgeon will cut the fibroids into small pieces in order to remove them from the uterus. If the fibroids are too large, the abdominal myomectomy procedure is performed, which involves making large incisions in the abdomen to remove the fibroids. The fibroids are pushed out through the vaginal or abdominal openings. Following the removal of the fibroids, the surgeon will remove the instruments, release the gas, and close the incisions with stitches.
