Chemotherapy remains a cornerstone in the treatment of many cancers. At Manipal Hospitals Mysuru, chemotherapy is administered through various specialised vascular access methods to ensure the safe and effective delivery of chemotherapy drugs.
Methods of Chemotherapy Access
A PICC line is a thin, flexible tube inserted into a large vein in the arm. It is used for medium-term chemotherapy where multiple cycles are planned over weeks or months. The procedure is done under ultrasound guidance and usually takes less than an hour. Once placed, a PICC line (Peripherally Inserted Central Catheter) allows for easy and repeated access to administer medications and draw blood, eliminating the need for multiple needle pricks. It also helps reduce the risk of vein irritation caused by potent chemotherapy drugs. Patients are provided with comprehensive instructions on how to care for the insertion site to help prevent infection, especially during home-based or daycare chemotherapy at our Mysuru centre.
A central line is placed in a large vein in the chest or neck for prolonged chemotherapy. It is ideal for patients who require frequent infusions, transfusions, or nutritional support. This catheter provides reliable venous access and reduces discomfort from repeated cannulations. The central lines are inserted using sterile techniques and are monitored regularly for patency and infection. It supports the safe and reliable delivery of complex drug regimens, particularly when high-dose or combination chemotherapy is required. Patients receive thorough education on central line care, including flushing techniques and dressing changes, to help prevent blockages, infections, or other complications throughout the course of treatment.
A chemo port is a small titanium or plastic device implanted under the skin, usually on the chest. It connects to a central vein and remains under the skin throughout the treatment period. The port is accessed using a special needle during each chemotherapy session. It is recommended for patients undergoing long-term therapy due to its durability and low maintenance. Chemo port placement is done surgically under local anaesthesia and requires only a small incision. Once healed, it allows easy, repeated access with minimal skin damage and is compatible with both inpatient and day-care chemotherapy protocols.
This procedure is used to examine the condition of bone marrow before, during, or after chemotherapy. It helps assess cellularity, detect abnormal cells, and monitor treatment effectiveness, especially in blood cancers. In Mysuru, the test is typically performed from the hipbone using a fine needle. Local anaesthesia is applied to reduce discomfort. A small sample of bone marrow is withdrawn for examination under a microscope. This procedure plays a crucial role in confirming the diagnosis and may also be used to adjust or modify chemotherapy plans. The results help guide future treatment cycles and assess the body's response to the administered drugs.
Chemotherapy treatment involves multiple procedures designed to ensure safety, accuracy, and continuity of care. The chemotherapy treatment process includes individualised plans based on cancer type, drug tolerance, and patient health. Equally important is the management of chemotherapy side effects, which includes regular blood tests, medication for nausea and fatigue, and nutritional support. Together, these protocols help patients complete treatment with minimal complications and improved quality of life.
