Please note that availing the health checkup is completely optional and is a direct arrangement between the Employee and the respective Hospital/Clinic/Health-related service provider (hereinafter “Partner”). Only Employees of Infosys ("Infoscions") are eligible to avail the discounts offered by the Partner/Vendor. The Partner/Vendor will be seeking your personal details from you to schedule health check-ups at the kiosk or on their webpage. Employees are requested to read and understand the Partner/Vendor’s Privacy policy before registering and providing personal data. All services availed shall be at the Employee’s own cost, and Infosys shall not be liable for any reimbursement of any amount. Infosys shall not be liable for any losses, claims, effects, reactions, or outcomes resulting from the use of these offers or benefits, and there will be no involvement of Infosys Limited. The registration form and all associated data will be managed solely by the hospital, and Infosys will not have access to any personal information submitted, except for the Employee ID and the footfall count, which may be shared by the hospital with Infosys after the health camps completion for further analysis and required interventions purposes. Such data will be accessible to authorized SPOCs from Infosys HR and will be stored on Infosys internal servers located in India. Once the purpose has been fulfilled, your data will be safely deleted from our and vendor systems. Infosys adopts reasonable and appropriate security practices and procedures, including administrative, physical security, and technical controls, to safeguard your Personal Data.
