A living-donor liver transplantation occurs when a living individual donates a portion of his/her healthy liver. In the case of a living donor, the surgeries on the donor and recipient occur simultaneously or in close succession to minimize the time the liver segment is outside the body. For a living donor hepatectomy, the surgeon makes incisions on the abdomen and removes a portion of the liver (usually the right lobe). The surgeon then re-connects the blood vessels, bile ducts, and hepatic artery and closes the incision. Today, robotic approach is highly preferred as it requires smaller incisions to carry out the excision of the liver portion, which also translates faster recovery

for the donor. In the recipient surgery, the diseased liver is carefully removed and the healthy liver portion is transplanted. The blood vessels, bile ducts, and hepatic artery are reconnected to the circulatory and digestive systems.

A liver transplant surgery takes between 8 and 12 hours. Both donor and recipient are hospitalised in the intensive care unit for several days. Post-transplant, the donor's liver regrows to its complete size over a short duration, typically within days or weeks, though it can sometimes take several months. The donor might be a family member, such as a parent, sibling, spouse or adult child. Regular medical check-ups are a must to ensure optimal function of the liver.

There are two types of transplants:
a) Cadaveric (Deceased donor) liver transplant: Consent is obtained from a deceased donor's family, and the whole liver is procured and transplanted into the recipient.
b) Living donor liver transplant: A healthy person from the patient's family donates part of the liver and is transplanted into the recipient. The liver grows to normal in the donor as well as the recipient.

The waiting time for a donor's liver is three to four months. The actual waiting time depends on the number of people on the transplant list and, most importantly, the patient’s health. A patient who is more sick will automatically move higher on the list to receive their transplant faster.

Each transplant unit has its own set of rules for waiting on the transplant list and being notified when a donor organ becomes available. You are likely to be alerted via call/text/email when an organ becomes available.
